From the West (traveling eastbound)

Southern State:
Exit 43A - Central Islip (County Road 17 north) From the exit ramp, bear to the right onto Carleton Avenue North.  About a quarter mile after road narrows to two lanes you will see a Home Depot/ Target Shopping Center on your left.  At the traffic light just after the shopping center make a left onto N. Research Place, and a right into the UCP Health Center parking lot. 

L. I. E.:
Take the Long Island Expressway (I-495) east to Exit 56, Route 111. From the exit ramp, turn right at the first traffic light onto Route 111. Immediately bear to the left onto Wheeler Road/County Road 17. Wheeler Road turns into Carleton Avenue. Continue on Carleton Avenue for approximately 2½ miles and turn right onto N. Research Place and a right into the Health Center Parking lot.

From the East (traveling westbound)

Take the Long Island Expressway (I-495) west to Exit 56, Route 111. From the exit ramp, turn left at the first traffic light onto Route 111. Immediately bear to the left onto Wheeler Road/County Road 17. Wheeler Road turns into Carleton Avenue. Travel south on Carleton Avenue for approximately 2½ miles, turn right onto N. Research Place and a right into the Heath Center Parking lot.

Sunrise Highway:
Take Sunrise Highway (Route 27) to Carleton Avenue North exit. From the exit ramp, bear right onto Carleton Avenue/County Road 17. Travel north on Carleton Avenue for approximately 1½ miles and turn left onto N. Research Place and a right into the UCP Health Center Parking lot.

By Public Transportation

The UCP Health Center is a 25-minute walk south of the Central Islip LIRR station (Hicksville-Ronkonkoma branch). 

Suffolk County bus route 3C (Central Islip-Bay Shore) runs to the campus of NYIT. Just west of NYIT, across Carleton Avenue is the UCP Health Center.
